A Case of Extrapontine Myelinolysis without Central Pontine Myelinolysis

Abstract
A case of extrapontine myelinolysis without central pontine involvement was reported in a 28-year-old female with hyperemesis hypergravidarum. The patient displayed quadriplegia, ataxic gait, orolingual dystonia and psychotic regressive behavior.
The
patient was treated with diazepam (5mg/day) throughout the hospitalization period. Symptoms gradually improved after the 20th day of hospitalization. Six weeks prior to hospitalization, brain magnetic resonance imaging (MRI) revealed symmetric
high-signal intensity areas on proton-weighted and T2 - weighted images as well as low - signal intensity areas on T1 - weighted images of the caudate and lentiform nucleus. No foci of central pontine myelinolysis were present. The patient had
symmetric
basal ganglia abnormalities, which were consistent with extrapontine myelinolysis. On the 40th day of hopitalization, MRI showed partial dsappearance of the lesions in the basal ganglia and the patient's symptoms ignored.
